If you have a plaster teeth model that was made by our lab, fOLLOW THESE INSTRUCTIONS

we do not accept plaster teeth models that were not made by us at our lab. we will only accept models that we made for you from a previous order.

1. Shop for your custom-fit mouthguard.
2. Customize your mouthguard online in less than 10 minutes.
3. During checkout, choose I Have My Own Mold/Impression.
4. Check your emails. We will send you specific instructions on how to ship your plaster teeth model to our lab.

NOTE: Be advised that if we receive a mold that is damaged, or was not made by our lab from a previous order, we will not be able to use it.

DO NOT ship us a mold if the mouthguard has not been ordered yet. You MUST place your order first, then follow the instructions sent to you in a follow up email.

2. Customize your mouthguard online in less than 10 minutes.
3. During checkout, choose I Have My Own Mold/Impression.
4. Download the Mold Return Form below and enclose it with your mold. If you don't have a printer, just write down the required information on a piece of paper.
5. If there is no damage to the mold (chipped or broken teeth, cracks, etc.), you are not required to email us photos of your mold. Be advised that if we receive a mold that is damaged, or was not made by our lab, we will not be able to use it.
6. Package your mold carefully when shipping it to our lab...they are fragile! Once we receive it, production of your mouthguard will begin the following business day.
7. DO NOT ship us a mold if the mouthguard has not been ordered; you MUST include a completed Mold Return Form with your order and contact information.
